Mentre la relazione tra George e Callie procede molto bene suscitando la curiosità di Izzie, tra Derek ed Addison le cose, ed in particolare il sesso, non vanno come dovrebbero.
Un famoso violinista, idolo di Burke, vorrebbe che gli fosse tolto il pacemaker impiantatogli dallo stesso Burke qualche tempo prima, perché non gli permette di tenere il ritmo e di suonare bene. Il cardiochirurgo però è molto preoccupato e sconsiglia il musicista ma questi gli dice che si rivolgerà ad un altro medico se lui non vorrà farlo. Burke è teso ma poi si convince ad eseguire l'intervento durante il quale purtroppo il violinista muore. Preston è distrutto e Cristina tenta di consolarlo.
Alex è assegnato contro la sua volontà ad Addison per tutta la giornata, ed insieme devono fare un cesareo ad una donna che ha già altri sei figli. La donna chiede loro di legarle le tube durante l'intervento senza che però il marito, fervente cattolico, lo venga a sapere. Lei non ce la fa più e non vuole avere più figli. Addison la accontenta fingendo delle complicazioni durante il cesareo ma Alex mette al marito la pulce nell'orecchio e l'uomo vuole fare causa all'ospedale. Addison prova a convincere la donna a dire al marito che è stata lei stessa a chiedere di fare in modo di non poter avere più figli, ma lei si rifiuta per non rovinare il suo matrimonio, lasciando così la dottoressa Sheppard in una brutta situazione professionale. Per vendicarsi Addison chiede alla dottoressa Bailey di assegnare Alex al suo reparto fino a tempo indeterminato, visto che a lui la cosa non fa affatto piacere.
Per la giornata la dottoressa Bailey si occupa di Danny insieme a Meredith, si accorge così che tra lui ed Izzie c'è qualcosa che, naturalmente, lei non approva. Meredith dunque si sente in dovere di avvisare sia Izzie che lui di stare attenti. Danny vuole essere staccato dalle macchine per potersi alzare anche se la procedura è un po’ delicata. Dopo un iniziale momento di difficoltà, i macchinari portatili a batterie funzionano perfettamente. Finalmente lui ed Izzie possono abbracciarsi.
La Bailey comunque non si sente ancora reinserita e se la prende con Webber il quale le spiega di stimarla sempre molto, ma le vuole dare un po’ di tempo per riprendersi dalla maternità.
Izzie e George sono a neurologia a seguire una paziente di Sheppard con problemi di epilessia. Derek la vuole fuori il prima possibile perché la donna, avvocato divorzista di grido, continua a fare delle insinuazioni sul suo rapporto matrimoniale. George ed Izzie hanno dunque il compito di farle venire un attacco per localizzare la parte del cervello dove eseguire un intervento, ma dopo videogiochi, caffeina, televisione, non ottengono nulla. I due amici battibeccano in continuazione perché Izzie vorrebbe sapere dove George passa la notte e lui invece le rinfaccia di avergli dato un consiglio sbagliato riguardo Meredith. Di fronte all'ennesima lite dei due specializzandi, la donna ha una crisi. Capiscono così che forse è proprio a causa dei tanti litigi ai quali la donna deve assistere, che gli attacchi si scaturiscono. La donna decide allora di cambiare stile di vita prima di optare per l'intervento.
Finn, il bel veterinario, chiede a Meredith se è single e se vuole uscire con lui, ma lei all'inizio dice che non può uscire con nessuno e che lavora a maglia. Più tardi ci ripensa e va da lui e i due passano la serata a fare partorire una cavalla. Finn le propone poi di rimanere per cenare e fare una doccia ed anche qui lei prima dice no e poi sì. Nel frattempo Doc, il cane, sta di nuovo male e Derek lo porta da Finn. La faccia che fa Sheppard vedendo Meredith scendere le scale del veterinario con i capelli bagnati, è abbastanza eloquente. Così torna a casa e prova a sistemare le cose con Addison proponendole "sesso bollente".
La divorzista prima di andarsene fa notare a George che quando due litigano come hanno fatto lui ed Izzie è perché si vogliono molto bene. Lui decide allora di tornare nella casa di Meredith e invita anche Callie a passare la serata con loro.

Callie ha passato la notte con George e al mattino entra in bagno seminuda mentre Meredith ed Izzie stanno lavando i denti. Le ragazze rimangono stupefatte anche perché la dottoressa Torres non si lava le mani dopo aver fatto pipì di fronte a loro. Izzie informa George dell'accaduto e lui non sa cosa fare, raccomanda a Callie di lavarsi le mani e lei gli racconta che si era lavata in cucina. Rimane ferita da George e le dispiace che non l'abbia difesa. Anche Burke è ferito, Cristina si è addormentata mentre facevano sesso e lei non può che difendersi dicendo che aveva dormito molto poco.
Meredith continua ad uscire con Finn, lui è un tesoro e le fa delle domande sulla sue vita privata dicendole che è stata sicuramente ferita e spaventata, ancora non sono stati a letto insieme.
Un grave incidente stradale porta al Seattle Grace Hospital il ragazzo che l'ha provocato e una famiglia composta da genitori, figlia incinta con marito.
Ad un primo esame tutti sembrano stare abbastanza bene, ma la tac alla giovane incinta evidenzia invece una situazione molto molto grave. Tutta l'equipe è chiamata ad operarla e ad un certo punto si decide per seguire una procedura che prevede un periodo di pausa durante l'intervento per non mettere ancora più a rischio la vita della ragazza. Devono attendere che la ragazza riprenda un po’ le forze prima di continuare.
Il ragazzo che ha provocato l'incidente è uno specializzando in chirurgia di un altro ospedale e Meredith si deve occuparsi di lui con la supervisione di Derek, il quale però è molto scortese e la tratta male.
Dopo un paio di sfuriate Meredith gli chiede perché ce l'ha col lei e lui in pratica le da della poco di buono perché l'ha vista a casa di Finn. Lei risponde che è stato lui a mollarla e lei sta solo cercando di rimettersi in piedi dopo il grande dolore che ha provato per aver dovuto rinunciare a lui. Non ha alcun diritto di giudicarla o di essere geloso, ma Derek le risponde solamente che tra loro adesso è veramente finita.
Danny ha una crisi perché si è sforzato troppo nel fare le scale e dopo si sfoga con Burke ed Izzie perché non ce la fa più a stare in ospedale. La sua situazione è precaria, Izzie tenta di tirargli su il morale ma lui le dice di sentirsi frustrato. Lei allora si stende sul letto con lui per fargli un po’ di compagnia e coccolarlo, rischiando però di mettersi nei guai perché i rapporti tra medici e pazienti non sono consentiti.
L'intervento alla ragazza ricomincia ma i chirurghi non riescono a salvarla. Alex, minacciato da Addison ha il compito di occuparsi del bambino e quando la ragazza non ce la fa, lui si prende la responsabilità di farle un cesario e salva almeno la vita del piccolo.
Il ragazzo che ha provocato l'incidente si sente molto in colpa, ha avuto un colpo di sonno dopo un turno di lavoro in ospedale troppo lungo, chiede di poter parlare con i familiari della ragazza per scusarsi. Il padre di lei va nella sua stanza e dopo un primo momento di incertezza lo perdona e piange con lui facendo commuovere anche George e Meredith lì presenti.
George rivolge la parola a Meredith per la prima volta dopo settimane, le chiede se si vedranno più tardi a casa.
Meredith dice a Finn che se lui conoscesse la sua storia deciderebbe di stare alla larga da lei, la sua famiglia è incasinata e anche lei lo è. Finn allora le racconta qualcosa di lui, orfano di madre, padre del tutto assente, vedovo: lui non ha detto di non essere impaurito e ferito, ma ora ha incontrato una ragazza carina a cui piacciono i cani, i puledrini e salva la gente per vivere, quindi forse per lui la fortuna sta girando. Meredith gli sorride e lo bacia con dolcezza.

Cristina: Ah, you're missing George.
Burke: No. No, of course not. Cristina: Drink your coffee.
Burke: Oh, no that's all right. Actually I'm waiting on--
George (Walking up with a cup): Ooh, cappuccino.
Burke: Hey, O'Malley!
George: Big news, Eugene Foote's here he's having big problems with his pacemaker.
Burke: Eugene Foote is here?
George: Uh-huh.
Burke: In this hospital?
George: Uh-huh.
Cristina: Who's Eugene Foote?
George: Eugene is Burke's hero. Burke flew down to San Francisco last year to put in his pacemaker. Burke has like 40 of his albums.
Burke: Uh, no 42.
George: Actually 43 because you just got the greatest hits CD with the DVD.
Burke: Ooh, right. You want in on Foote?
George: Yes, but I'm on neuro today with Dr. Shepherd. (Leaves)
Cristina: Uh, I want in. Hello! I want in.
Burke: Hmm. Yeah. Sure.
Cristina: Burke I laid on top of you naked last night so why don't you act nostalgic about that.
Izzie: I'm just curious, George. Curious George. (Laughs) Get it? (He ignores her) Nevermind. All I'm asking is where do you live?
George: Listen to this, there's an old school arcade game that's been known to enduce seizures if you reach level 53.
Izzie: And most lawyers don't play an arcade game for 53 levels. Oh and George, by the way, um...where do you live?
George: I'm busy doctoring, Dr. Stevens. No time for chit-chat.
Callie (Appears in the doorway): Mornin' Dr. O'Malley.
George: Mornin' Dr. Torres.
(She comes over and they talk, and Izzie is watching)
Izzie: Dr. O'Malley how's all the doctoring going?

Callie ha passato la notte con George e al mattino entra in bagno seminuda mentre Meredith ed Izzie stanno lavando i denti. Le ragazze rimangono stupefatte anche perché la dottoressa Torres non si lava le mani dopo aver fatto pipì di fronte a loro. Izzie informa George dell'accaduto e lui non sa cosa fare, raccomanda a Callie di lavarsi le mani e lei gli racconta che si era lavata in cucina. Rimane ferita da George e le dispiace che non l'abbia difesa. Anche Burke è ferito, Cristina si è addormentata mentre facevano sesso e lei non può che difendersi dicendo che aveva dormito molto poco.
Meredith continua ad uscire con Finn, lui è un tesoro e le fa delle domande sulla sue vita privata dicendole che è stata sicuramente ferita e spaventata, ancora non sono stati a letto insieme.
Un grave incidente stradale porta al Seattle Grace Hospital il ragazzo che l'ha provocato e una famiglia composta da genitori, figlia incinta con marito.
Ad un primo esame tutti sembrano stare abbastanza bene, ma la tac alla giovane incinta evidenzia invece una situazione molto molto grave. Tutta l'equipe è chiamata ad operarla e ad un certo punto si decide per seguire una procedura che prevede un periodo di pausa durante l'intervento per non mettere ancora più a rischio la vita della ragazza. Devono attendere che la ragazza riprenda un po’ le forze prima di continuare.
Il ragazzo che ha provocato l'incidente è uno specializzando in chirurgia di un altro ospedale e Meredith si deve occuparsi di lui con la supervisione di Derek, il quale però è molto scortese e la tratta male.
Dopo un paio di sfuriate Meredith gli chiede perché ce l'ha col lei e lui in pratica le da della poco di buono perché l'ha vista a casa di Finn. Lei risponde che è stato lui a mollarla e lei sta solo cercando di rimettersi in piedi dopo il grande dolore che ha provato per aver dovuto rinunciare a lui. Non ha alcun diritto di giudicarla o di essere geloso, ma Derek le risponde solamente che tra loro adesso è veramente finita.
Danny ha una crisi perché si è sforzato troppo nel fare le scale e dopo si sfoga con Burke ed Izzie perché non ce la fa più a stare in ospedale. La sua situazione è precaria, Izzie tenta di tirargli su il morale ma lui le dice di sentirsi frustrato. Lei allora si stende sul letto con lui per fargli un po’ di compagnia e coccolarlo, rischiando però di mettersi nei guai perché i rapporti tra medici e pazienti non sono consentiti.
L'intervento alla ragazza ricomincia ma i chirurghi non riescono a salvarla. Alex, minacciato da Addison ha il compito di occuparsi del bambino e quando la ragazza non ce la fa, lui si prende la responsabilità di farle un cesario e salva almeno la vita del piccolo.
Il ragazzo che ha provocato l'incidente si sente molto in colpa, ha avuto un colpo di sonno dopo un turno di lavoro in ospedale troppo lungo, chiede di poter parlare con i familiari della ragazza per scusarsi. Il padre di lei va nella sua stanza e dopo un primo momento di incertezza lo perdona e piange con lui facendo commuovere anche George e Meredith lì presenti.
George rivolge la parola a Meredith per la prima volta dopo settimane, le chiede se si vedranno più tardi a casa.
Meredith dice a Finn che se lui conoscesse la sua storia deciderebbe di stare alla larga da lei, la sua famiglia è incasinata e anche lei lo è. Finn allora le racconta qualcosa di lui, orfano di madre, padre del tutto assente, vedovo: lui non ha detto di non essere impaurito e ferito, ma ora ha incontrato una ragazza carina a cui piacciono i cani, i puledrini e salva la gente per vivere, quindi forse per lui la fortuna sta girando. Meredith gli sorride e lo bacia con dolcezza.

Husband: She's mad at you.
George: I, I didn't know you were awake.

George: Uh, has anyone talked to you about your wife? About what's happening?
Husband: What's happening. Yeah, uh, I can't really wrap my brain around whats happening. You know, I'm from here, Seattle, and uh I go away to college and I come back married to this big-haired, drawling, southern girl. And everyone I know thinks I'm crazy, but I love her. And I just, I love her. And, uh, what's happening is that my big-haired southern girl could be dying and I can't think about that. You know, I need to think about something else. Something that doesn't matter so much, something that doesn't have me burying my 22 year-old wife and baby. So, so, what the hell did you do?
George: I believed my friend when she told me that Dr. Torres didn't wash her hands.
Husband: Well, I hope that's not true, seeing as she just performed surgery on me and all.
George: Yeah.
Meredith: I never should have told you about George.
Derek: No, its fine, I'm glad I know about him, and the vet. You really get around.
Meredith: What did you just say to me?
Derek: Its unforgivable.
Meredith: I don't remember ever asking you to forgive me.
Derek: So was the knitting a phase? Who's next? Alex? 'Cause I hear he likes to sleep around too. You two have that in common.
Meredith: [she grabs him as he turns to walk off] You don't get to call me a whore. When I met you, I thought I had found the person that I was going to spend the rest of my life with. I was done. So all the boys and all the bars and all the obvious daddy issues, who cared, because I was done. You left me. You chose Addison. I'm all glued back together now. Finn: My mother's dead. She got cancer when I was ten and she suffered for a really long time and then she died. And my father never recovered. Its kind of like he died with her, except that his body's above ground and permanently placed in front of a TV with a bottle of scotch in his lap. And the last woman I slept with was my wife, but she died too. It was a car crash so it was quick. She didn't suffer, which I appreciated.
